Why didn't an approved customer get tax-free checkout?
Confirm two things: (1) Ensure customer accounts are enabled in Shopify Settings > Customer accounts; (2) In app Settings, verify that 'Auto-Exempt on Approval' is toggled ON. When approved, Shopify automatically sets customer.taxExempt = true.
